Name Change

If your passport was issued less than 1 year ago and your name has been legally changed due to marriage or court order:

  • Bring your current, valid U.S. Passport
  • Complete Form DS-5504: Application for a U.S. Passport: Name Change, Data Correction, and Limited Passport Book Replacement 

Please note that the Embassy will only accept applications that have been filled out online and printed on one-sided pages.

    • The original marriage certificate or the government-issued document evidencing your legal name change
    • Bring two (2) identical photos (2×2 inches, white background).
    • No fee
    • The application will be submitted in person. Complete a request for an appointment at the following: Department of State Website.
    • Allow approximately 10 working days for processing. Passports must be collected at the American Citizen Services (ACS) section by the applicant or an authorized representative. An appointment is not necessary to pick up the passport; however, collection must be during office hours. The applicant or authorized representative must provide the current passport of the applicant for cancellation in order to receive the newly issued passport.
    • A person may be designated as an authorized representative by providing a written statement with the following information: applicant name, passport number of applicant, statement of authorization, name of authorized person, and a signature of the applicant. The authorized representative should bring this statement, identification, and the expiring passport of the applicant, if applicable, to collect the applicant’s passport.

If your passport was issued more than 1 year ago and your name has been legally changed due to marriage or court order:

  • Your current, valid U.S. passport
  • Completed Form DS-82, Application for a U.S. Passport 

Please note that the Embassy will only accept applications that have been filled out online and printed on one-sided pages.

    • Bring two (2) identical S.-sized passport photos (2×2 inches, white background).
    • The original marriage certificate or the government-issued document evidencing your legal name change
    • The fee is $110 payable in dollars or the equivalent in Tanzanian Shillings, USD or international credit card (VISA, MasterCard, Dinners, American Express, and Discover).
    • The application will be submitted in person. Complete a request for an appointment at the following: Department of State Website.
    • Allow approximately 10 working days for processing. Passports must be collected at the American Citizen Services (ACS) section by the applicant or an authorized representative. An appointment is not necessary to pick up the passport; however, collection must be during office hours.
